The totality of information and documentation you offer will be considered in evaluating an application, and, unless expressly required by law, the student will not be denied enrollment based on your inability to provide certain form(s) of documentation where other acceptable evidence is presented.

Please be aware that any initial determination of the student’s eligibility to attend school in this district is subject to more thorough review and subsequent re-evaluation, and that tuition may be assessed in the event that an initially admitted student is later found ineligible. If your student is found ineligible, now or later, you will be provided the reasons for our decision and instructions on how to appeal.

Kindergarten - 12th Grade

Online Registration Before starting the online registration process, please be sure to have the following documents ready to be uploaded. The initial registration is on Genesis and you will need:

• Student's Birth Certificate
• Student's Immunization Record
• Medical History
• Proof of Physical - Universal Child Health Record
• NJ Department of Health Minimum Immunization Requirements for School Attendance
• Parental Permission to Release Information
• Parent/Legal Guardian Certification - Needs to be notarized (a notary is available at the BOE office if needed)
• NJSIAA Student Athlete Residency Affidavit - 9th - 12th grade only. Needs to be notarized (a notary is available in he BOE office if needed)
• Medicaid (SEMI) Form
• Parental Permission to Release Information

• Homeowner/Host Certification - To be completed by the Florence Township homeowner/host when the child and their parent/legal guardian are residing with that homeowner/host in Florence Township without a written lease and without paying rent.
• Landlord Certification - To be completed by the landlord when the child and his parent/legal guardian are residing in a dwelling managed by that landlord in the Florence Township School District without a written lease.
